Creating a Facebook Lead Form

🛠️ Step-by-Step: Create a Lead Form

1. Choose Form Type

Facebook will first ask you to select a form type:

  • More Volume (Recommended): Simpler, faster experience for users; better for lead quantity.

  • Higher Intent: Adds a review step; better for filtering more serious leads.

✅ Choose “More Volume” if your goal is to generate as many leads as possible quickly.

2. Form Name

Give your form a recognizable name. Example:

21-Day Challenge - Summer 2025 - GymName

This is for internal use only and won’t be seen by users.

3. Introduction (Optional)

You can include a brief description of your offer. This section is optional but helps set context.

Headline Example:

Ready to Transform Your Body in Just 21 Days?

Description Example:

Claim your spot in our limited-time challenge—fun workouts, expert coaching, and real results!

4. Questions

These are the fields you’ll collect from the user.

Recommended Fields:

  • First
  • Last
  • Email
  • Phone Number

You can add custom questions (e.g., multiple choice or short answer), but keep it simple for better conversion.

5. Privacy Policy

You must link to your business’s privacy policy.

  • Add the URL to your Privacy Policy page.

  • Optionally, include a disclaimer or message about how their information will be used.

Example:

We’ll use your information to contact you about your free consultation.

6. Completion Screen

This is the final screen people see after submitting the form.

Headline Example:

rust

CopyEdit

You're In!

Description Example:

sql

CopyEdit

We’ll be in touch within the next 24 hours to schedule your first visit. Get excited! 💪

You can also add a call-to-action button such as:

  • “Call Us”

  • “View Website”

  • “Book Now”

Link it to your gym website, scheduling tool, or direct phone call.

✅ Final Steps

  • Click “Publish” to save the form.

  • You can now select this form while setting up or editing your lead ad.

⚠️ Note: Once a form is published, it cannot be edited. If you need changes, duplicate the form and edit the copy.

📌 Best Practices

  • Keep it short—3 fields max.

  • Use “More Volume” form type for cold traffic.

  • Always double-check your Privacy Policy link.

  • Include a strong thank-you message or CTA on the Completion Screen.
